*Attendant Care services involve hands-on assistance for aging adults and persons with disabilities. These Services are provided to allow aging adults or persons with disabilities to remain in their own homes and to carry out functions of daily living, self-care, and mobility. Provision of assistance with personal care, assistance with mobility, Assistance with elimination, assistance with nutrition preparation of meals, assistance with transfer, safety, reminding individuals to self-administer medications. Escorting individuals to community activities that are therapeutic in nature or that assist with developing and maintaining natural supports, and assistance in doctors' appointments.

CLIENT ELIGIBILITY FOR HOME CARE SERVICES

A client must be needing assistance and homebound and the client has a need for assistance to leave home.

*Aged and Disabled (A&D) Waiver - allows individuals who are aged, blind, or disabled to remain in their home as an alternative to nursing facility placement. Our Agency is certified by the Indiana Family and Social Services Administration, Division of Aging (FSSA/DA). Home and Community-Based Services Waivers.

REFERRAL SOURCE FOR HOME CARE SERVICES.

 

The client's attending physician can make referrals for home care services.

​

Upon client admission, a plan of service should be developed within 48 hours for the physician's signature if the client is coming from the hospital or rehabilitation facility.

​

We coordinate the care of our clients with the physician who certifies the medical conditions for home care. Our staff closely monitor the client to ensure the quality of assistance is being rendered. 

 

Our referral source could be from families' primary caregivers, hospitals, nursing homes, assisted living, rehabilitation facilities, and other senior communities, and public social functions that cater to seniors.

We will assist/work with the primary family caregiver according to the plan of care after the assessment of the client.
